WORK SCHEDULE

• Full-time, 100% FTE (40 hours per week)

• Day Shift DEPARTMENT DESCRIPTION: UW Medicine’s (UWM) Transgender and Gender Non-Binary Health Program's primary goal is to reduce the health disparities experienced by the transgender and gender non-binary (TGNB) community by providing inclusive and patient-centered gender affirming healthcare access that is integrated throughout primary, surgical, specialty, and mental health services. The TGNB Health Program strives to ensure equity, diversity, and inclusion principles inform all system-wide quality improvement processes and practices, and incorporates TGNB community voices and health equity measures into program development. • Opportunity to provide career growth PRIMARY JOB RESPONSIBILITIES:

• Review and prioritize incoming referrals from multiple sources using their knowledge of each physician’s specialized skill set

• Triage incoming referrals to assure the appropriate specialist will be reviewing referrals

• Handle intake of each referral requests, ensuring complete information is received and entered into the electronic medical record system in a timely manner

• Coordinates appointments for the patient at the appropriate facility and assists with urgent insurance authorization or financial assistance

• Manage and resolve a high volume of phone inquiries and messages

• Review appointment follow-up requirements and expectations with patients. Educate them to ensure adequate understanding of the purpose of each appointment and follow-up recommendations

• Offer financial assistance to self-pay and underinsured patients

• Serve as a primary program contact to other departments, patients, providers outside facilities REQUIREMENTS:

• High school graduation or equivalent AND two years of experience in the program specialty

• Three years of Patient Support Service experience OR
